Consumer Directed Services Coordinator
Job Summary: The CDS
Coordinator assists persons with physical, mental, or
multiple disabilities in accessing consumer-directed
personal attendant services. The CDS Coordinator is
responsible for accurate reporting of services to
MOHealthNet, MO DHSS, and providers of support services.
The Coordinator provides care planning services, skills
training, and information and referral services. The
Coordinator advocates for the needs of persons with
disabilities, documents services provided, makes
community presentations and may supervise individuals
involved with the CDS Program.
Qualifications: Personal experience with a
disability preferred or experience working with
individuals with disabilities. Bachelor’s degree in
social work, psychology, or other related field to
independent living. Experience in a counseling,
training, or related position within a human service.
Educational qualifications may be waived for equivalent
experience.
