Transportation Fees & Forms


 

Mandatory Fare Structure: All fares will be according to the current policy as listed below. For transportation outside of the service area, please call the office at (573) 556-0400 or (877) 627-0400. All passengers are required to have a bus pass. If the rider does not have a pass, the driver can refuse service. Bus passes will be available at the ILRC office. Drivers will not sell tickets on the buses.

 
 

Fare Structure - Effective 6/1/08

 

 

Drivers will make 1 punch each time a consumer boards the bus.

  $  4.00 for a one-way trip
  $  8.00 for a round trip
  $  4.00 each additional stop
 
Travel from one county to the next will be two punches each time a consumer boards.
 
  $  8.00 for a one-way trip
  $16.00 for a round trip
  $  4.00 each additional stop
 
Travel into a third county will be three punches each time a consumer boards.
 
  $12.00 for a one-way trip
  $24.00 for a round trip
  $  4.00 each additional stop
 

 

Consumers must make arrangements for all planned stops when making a ride request.

If a consumer tells the driver they need to make an additional stop or a different stop after boarding, the driver will refuse this change and will follow the ride request made with the ILRC office.

All ILRC bus passes are non-refundable, no exceptions. Passes for trips may be applied to alternate trips or to an ILRC bus pass.

** You may purchase ILRC Transportation Passes during the following hours:

 

Monday through Friday from 8:00 a.m. - 5:00 p.m.

 

** ILRC will accept Cash, Check (with valid state ID number only), Money Order or Credit/Debit Cards (MasterCard and VISA only) for ILRC Transportation Pass Sales. Passes may be purchased by phone with a credit/debit card or by mail with a check or Money Order.

 


 

Forms



 

Consumer Transportation Agreement

This form states that you have read, or have had read to you, the ILRC Policies and Procedures, and that you  agree to follow them.

 

ILRC Transportation Waiver

This waiver states that ILRC is not held responsible for injury incurred if you use the lift mechanism on the ILRC Transportation Buses without the aid of a wheelchair.

 

Transportation Intake Form

This is the basic intake form you fill out to request Transportation Services. This form provides ILRC with information necessary to render services to an individual.

Notice:
Everyone who rides the ILRC buses will be required to read, or have read to them, the policies and procedures that cover conduct on the ILRC buses, and will need to sign the agreement form stating that they understand and will adhere to those policies and procedures while participating in the program.

Anyone refusing to sign the policies and procedures will not be allowed to use the ILRC Transportation Program, regardless of monies paid for ILRC Bus Passes. Those monies will be forfeited to ILRC.

ILRC Reserves the right to stop ILRC Transportation services to any consumer who violates the ILRC Transportation policies and procedures regardless of monies paid for ILRC Transportation Bus Passes. Those monies will be forfeited to ILRC.
